What services do you offer?
Our physician-led post-acute specialty programs include orthopedic rehabilitation, post-surgical care, cardiac and pulmonary reconditioning, stroke rehab, diabetes management and education, as well as comprehensive wound care and many more. We offer respiratory therapy services and COPD programs that are a cut above those available from any other Skilled Nursing Facility in the area.
For those in need of long-term care or Alzheimer’s / memory care, CareOne’s local communities offer a complete spectrum of care options. Respite care offers a safe, homelike environment for those you love when the primary caregiver is not available.

Can you explain what rehab usually entails?
Upon admission, each patient receives a complete evaluation from a multidiscipline team, which will include nursing, physical therapy, occupational therapy, and a physiatrist (MD specialized in rehab medicine). Speech therapy is also available if indicated. An individual plan of care will be formulated to meet each patient’s specific needs. Therapy is available 7 days a week. We keep the patient and family up to date on progress, including the patient in the setting of goals. Finally, social work will assist in assuring that whatever is needed upon discharge is in place.
Will I be followed by a doctor?
CareOne has a large staff of credentialed physicians of various specialties available to follow each patient. Each patient is assigned a medical doctor who will oversee the medical care as well as a physiatrist who will oversee the rehabilitation therapy. Assistance will be given in arranging office visits to physicians who do not frequent the facility.
